In July 2023 we began the process of merging with UMCH Family Services. This process has been strategic and slow to give us time to navigate relationships and legal changes with all providers and stakeholders.
Effective January 1, 2025 CHOICES EIN has been dissolved, and we now are one organization operating under the UMCH Family Services Federal ID #31-4380047.
What does this mean for you? You will need to use this ID number for all tax deductible charitable contributions made on or after January 1, 2025. We are maintaining our dba of CHOICES Inc, you can still refer to us as CHOICES and make check payments to CHOICES Inc.
Many of you may also use 3rd party platforms for matching gifts or choose to give through a Foundation. For those entities you will need to use our new EIN and our legal name is:
