Mental Health Intake Assessor

CHOICES Inc. is a tight-knit, strengths based organization that brings change to youth and families.  CHOICES Inc. invests in each employee and recognizes the need for a work life balance.  The Mental Health Intake Assessor is part of the CHOICES Home Based Services team in Dayton Ohio.  The Mental Health Intake Assessor serves clients in need of mental health services.  The Mental Health Intake Assessor is responsible for completing mental health assessments, mainly virtually and maintaining their schedule to meet productivity expectations.

Job Responsibilities (include, but are not limited to)

  • Utilize clinically appropriate skills to complete mental health assessments based upon client need.
  • Provide individualized service planning
  • Create note/documentation for every session (daily as clients are seen) in the HER
  • Collaborate with internal team members and external treatment teams to provide a holistic service approach

 Job Requirements:

  • Master’s degree in counseling or social work required
  • Mental Health Counseling – minimum 5-year experience required
  • Licensed independent social worker (LISW) or licensed professional clinical counselor (LPCC) preferred
  • Knowledge of formulating diagnosis and case conceptualization
  • Knowledge of administration and scoring of traditional assessment measures, models and techniques, including clinical interviewing and mental status exam
  • Valid drivers license, auto insurance and reliable transportation required
  • Local travel required
